We are seeking Medication Management Nurse/LVN LPT to join our team in Long Beach!
Pay: Up to $35/hr
Hours: 35 HOURS A WEEK ; 8am to 6pm between M-F
Duration: Temporary (3+ months)
Location: 4 different location in the Long Beach area. Office setting with medication rooms, do not need to drive

Role Summary: The nurse will address health issues, attend to modifiable risk factors, and assist members in becoming engaged partners to improve overall health and well-being. This position will also act as a resource for the interdisciplinary team regarding management of physical healthcare needs and medications. Direct health care may take place in an office setting, members’ home, or on the street when working with homeless individuals.
Background Requirements:
- The nurses will not be working/providing care in your typical hospital/clinic type setting, but onsite at our various locations/offices in Long Beach

Responsibilities include but are not limited to:
- Dispenses medication to members according to physician prescription and in keeping with funding source requirements and professional standards and practices.
- Administer medications/injections as needed.
- Provides day-to-day oversight of medication room, including daily management of administrative tasks such as logs and other records, charting of services performed, compliance with protective technical polices and procedures, and other similar duties.
- Demonstrate responsibility for the proper transcribing, administration, monitoring, and managing of medications. Display knowledge of adverse side effects, indications for use, and contraindications of medications.
- Assist in the collection of health information. Monitor and manage health conditions by taking measurements of weight/BMI, blood pressure, glucose levels, and lipid profiles
- Develop collaborative relationship with psychiatrists, nurse practitioners, physical health care providers and interdisciplinary personnel to address complex needs of individual members
- Provide education to members regarding health promotion, medications, and disease management. Focus on skill building to enhance member’s ability to handle health concerns independently
- Educate Personal Service Coordinators and aid with medication assessments
- May provide information, coaching, counseling and training to members in the area of medication management, and/or provide other physical or mental health services to members in keeping with scope of license and practice.
- Participates in staff and community meetings and in the day-to-day operations of the program as needed.
- Maintain all records and documentation in accordance with County Contracted Provider, Carelogic system (EHR), Office of Diversion and Reentry Database, funding source standards and requirements, and documents in required program, including 24 hour turnaround documentation
