RN Case Management and Health Promotion

Communicate with your members via phone or secure messaging using your clinical expertise in an unscripted way to provide education on health conditions, new medications, and procedures

Assist with appropriate level of care, type of service, type of provider; assist with appointments or facilitating of service

Coordinates community resources with emphasis on medical, behavioral and social services.

Maintain communication with members via phone, message or email

Actively reach out to members undergoing difficult health challenges, develop care plans with them, and help guide them through the healthcare system to improve the overall experience

Demonstrates excellent observation, problem-solving, critical thinking, and verbal and written communication skills;

Self-directed and able to work with minimal supervision

Shows the ability to organize and prioritize workload independently

Work with non-clinical teammates in assisting with health concerns/conditions

Job Requirements

License in good standing 

Monday through Friday, No holidays or weekends
