RemX is partnered with a highly regarded healthcare organization to hire 2 Telecare RNs to work in fast-paced primary care clinics. This is a Temp position, 40 hours per week.
- Provide clinical assessment, serve as the patient and family advocate by communicating with attending physicians, long-term care facility staff, case managers, and others as necessary.
- Consistently exhibit behavior and communication skills that demonstrate company’s commitment to superior customer service, including quality, care and concern with all internal and external customer.
- Follow established clinical protocols/guidelines and provide appropriate patient instructions.
- Provide collaboration with LPN/LVNs.
- Document authorizations for vendors and/or outside services as appropriate.
- Instruct patients on procedural preparations.
- Provide health education to patients and their families.
- Document and maintain records of interactions.
- Complete medical documentation of assessment and disposition.
- Notify pharmacies of new prescriptions and authorizations of refills as ordered by the clinician.
- Complete documentation of end-of-shift report.
- Participate in quality monitoring process.
- Performs additional duties as assigned.
