Providing a variety of clinical, psychiatric, and social services to individuals with substance abuse issues who have unique and often complex health and social needs. The Substance Use Disorder Case Manager is responsible for assisting the in obtaining health care and related substance about resources.

What you will do:
- Deals directly with individuals with substance abuse issues
- Interviews client to determine their health care needs.
- Develops individual short term client service plans.
- Makes referrals
- Monitors the delivery and outcome of services provided to clients by medical facilities, social service agencies and community resources.
- Provides intensive case management.
- Assists clients in obtaining medical and financial entitlements and benefits.
- Helps clients utilize community resources, develop independent living skills, and become reintegrated into the community.
- Educates clients on good health habits and available services to assist with health needs.
Job Requirements:
What you need:
- Bachelor’s Degree in Social Work or a closely related field.
- An equivalency of training may be substituted as determined applicable by Civil Service.
- Verification of education must be provided before eligibility list goes into effect.
- Must have drivers license and reliable transportation.

Remx is seeking to assist our client who is a Local not for profit looking for Weekend shift Program Aide for immediate start.
This position will be onsite at Hotel Louisville. It has very recently been decided that this position will need to be staffed 24 hours a day. There are several now several shifts available!!!
POSITION HIGHLIGHTS
$17.00 hourly
WEEKENDS ONLY- Various Shifts Available!!! First, second and third shift!!!!
Training will likely be Monday through Friday for first 2 to 3 weeks.
Onsite at 120 E. Broadway

WHAT YOU WILL DO
Interactions with those who are homeless to provide encouragement and motivation for change while maintaining compliance with agencies policies.
Ensure facility are safe and well maintained by conducting rounds.
Address security concerns to onsite security and program management.
Oversee implementation of facility rules
Delegate chores.
Monitor sign in logs.
Check for presence of drugs and alcohol; protocol in place if identified. (Security is also site)
Note: Third Shift will in most cases only be conducting room checks and be in place for any type of emergency to act as a point of contact and initiate contact to the appropriate person. For example, if there is a leak or other repair needs immediate attention.
Job Requirements:
WHAT YOU HAVE
HSD or equivalent.
CNA certification, mental health education, or other related education preferred but not required.
CPR first aide training.
Excellent written and communication skills.
A combination of education, training, or experience that results in demonstrated competency to perform.
Successfully pass all required preemployment background and drug screening.
Computer Literacy.

Manages appointments for multiple providers and rooms at multiple locations.
Processes requests for referrals.
Collects and processes cash, check, and credit card payments.
